This one-time code covers the initial setup of the RPM device and the time spent educating the patient on how to use it correctly and what to expect from the program.
Initial Setup & Onboarding

CPT Code 99454

This monthly code covers the supply of the RPM device (e.g., blood pressure cuff, scale) and the daily collection and transmission of patient health data. It is billable after a minimum of 16 days of readings are transmitted in a 30-day period.
Device Supply & Data Monitoring

CPT Code 99457

This monthly code reimburses your practice for the first 20 minutes of interactive clinical staff time spent on care management. This includes time spent on data analysis, patient calls, and care coordination.
First 20 Minutes of Care

CPT Code 99458

This is an add-on code used for each additional 20-minute block of clinical time spent on care management in a single month, billed in conjunction with CPT 99457.
