HCPCS Code: G0438

This code is used for a beneficiary’s first-ever Annual Wellness Visit. It includes the initial Health Risk Assessment (HRA) and the creation of the first Personalized Prevention Plan. This code can be billed once in a patient’s lifetime.
Initial Annual Wellness Visit

HCPCS Code: G0439

This code is used for all follow-up Annual Wellness Visits after the initial G0438 visit. It covers updating the patient’s HRA, reviewing their functional ability and risk factors, and updating their Personalized Prevention Plan for the current year.
Subsequent Annual Wellness Visit

HCPCS Code: G0402

Billed for the one-time Initial Preventive Physical Examination (IPPE), which must occur within the first 12 months of a patient’s Medicare Part B enrollment. This visit is a prerequisite before a patient becomes eligible for their first Annual Wellness Visit.
